I am informed that UAB “ISMIRA” (hereinafter - the Agency) will handle my data for the purpose of interview organization and the employment contract signing.

I agree that the Agency will collect and process my personal data, which is required for

-interview organization Evaluation of my candidacy and initiation of the Agency Service Agreement signing

-invitation to participate in job interviews organized by the Company

The following data can be collected and processed for these purposes: -Curriculum vitae (name, surname, citizenship, education, qualification, work experience, other data necessary for the specific work place) -Contact information (telephone, e-mail, social networks)

I agree that the Agency will carry out checks in the state registers and / or in the Internet and will transfer my personal data in order to assess the eligibility of my candidacy.

I understand that if I am selected for a specific position, I will have to provide additional documents requested by the Employer (medical certificate, police clearance certificate, etc.) and I do not object to the Agency performing the verification, translation and transfer of such documents to my future employer.

To whom can my personal data be transferred?

-For the execution of the contract concluded by the Agency (provision of services) : To the Employer on whose initiative staff recruitment and interview is carried out

-Compliance with legal requirements : State institutions, law enforcement agencies

-Agency‘s legitimate expectations / rights protection : Courts, lawyers, attorneys, audit companies, IT professionals

-For the execution of the contract with you or the person seeking for an Employee : Consulates, legal entities providing passenger transportation services

-My request : Post/courier services, etc.

How long will the Agency handle my data?

-During the interview organization process;

-As long as it is indicated in this Consent or until this Consent is cancelled, depending on what will happen sooner.

What rights do I have and how can I implement them?

-The right to obtain access to my personal data processed by the Agency for familiarization and to receive copies thereof

-The right to correct, supplement, delete, and transfer my personal data processed by the Agency

-The right to restrict the processing of my data in the Agency

-The right to disagree or to cancel this consent

-Other rights that may arise from the General Data Protection Regulation

How do I implement my existing rights?

Submit a request, including your name, contact details, how you would like to receive a response and what right you want to use. The request is submitted:

-In the Agency‘s office, to the Head of the Agency and, in his/her absence, to his / her deputy, by submitting an ID card or passport, or

-By registered mail, by attaching a notarized copy of your ID card or passport, or

-By email info@ismira.lt sending your request from the same email address that had been used at the point of your application submission .

I agree that my data will be stored in the Agency for the purpose of invitation to the job interviews for 2 (two) years, whereas after signing Agency Service Agreement - for a period indicated in the Agency Data Protection Rules.

Where will the Agency store my data?

I am informed that my data will be stored:

-In the Agency Emails, Servers, “Breezy” Recruiting and Applicant Tracking Software.

-In the computer of the Agency employee responsible for organization of the personnel recruitment

I am informed that after the expiry of the above deadline if this Consent validity was not extended, the Agency will delete the data it holds about me from e-mails / computers / servers, however in order to delete such data from job search portals (if I submitted the application through them), I have to apply to the specific portal by myself to implement such actions.

I am also informed that this Consent will be stored during its validity period and for 2 years after the cancellation or expiration of this Consent on the basis of the legitimate interests of the Agency (in order to prove the fact of consent in case of disputes regarding its scope, etc. in pre-trial institutions or in court).

In case of additional questions I will contact the above mentioned Agency contacts or directly the Agency office.
